Tennessen Warning (Minn. Stat. 13.04, subd 2) The information provided in connection with your application for licensure or registration, as well as any information collected during any investigations conducted by the Minnesota Board of Animal Health, will be used to assess your eligibility and compliance with relevant regulations. This information will be maintained as part of a file by the Minnesota Board of Animal Health. Providing this information is voluntary; however, failure to provide the requested information may result in the inability to process your application, complete the investigation and/or maintain your licensure/registration. While some of the information you provide is classified by Minnesota law as public, other information (such as a Social Security Number) may be classified as not public/confidential. Data classified as private or not public/confidential will be accessible only to Minnesota Board of Animal Health employees who have a work assignment that requires access to the data, and to other entities authorized by law to receive the data. This includes, but is not limited to, law enforcement agencies, the U.S. Department of Agriculture, the Minnesota Department of Revenue, the Office of the State Auditor, and the Office of the Legislative Auditor. In the event of a legal action, not public information may be released to staff at the Minnesota Attorney General’s Office. Finally, private data about you may be released to third parties through your informed consent or pursuant to a valid court order.
